Practice Area

Our Practice area includes the villages of Kelvedon & Feering, Eathorpe, Messing, Inworth, Rivenhall End, Rivenhall, Coggeshall Hamlet and Skye Green. We shall automatically register anyone who lives within our area. If patients who live outside this area wish to join the practice, they can be registered at the Practice’s discretion, but there is no automatic right to register.

How to Register

If you wish to register with us, please call in at reception and ask to complete a registration form, or give us your medical card. You will be registered with the Practice but you may choose a particular doctor if you wish. We ask all our new patients to attend a new patients medical, where we can obtain your medical history and assess your likely future needs.

Non-English Speakers

Pre-registration with the practice

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
